How do I obtain images from my Radiology exams?
To ensure the privacy and security of your medical records, all image requests must be submitted in writing. This serves as documented proof of your request.
Step 1: Complete the Authorization Form
Download and complete the Hartford HealthCare Authorization for the Release of Medical Records form. If a legal representative is making the request, copies of appropriate documentation must be included.
Step 2: Submit the Form
You may submit the completed form using any of the following methods:
